Sister had a miscarriage...?

my sister had a miscarriage at 20weeks but the doctors in the emergency room said she was only 14 weeks and also said that the baby might have died two weeks earlier (i don't believe this because i went with her to an ultrasound appointment at 14 weeks) they finally told her that there was blockage in the umbilical cord. my question is what could cause the blockage?
